Complete Intake Form

After you complete the checkout process you will receive an email to begin your journey with us by completing our comprehensive intake form. This helps us understand your health background and goals, ensuring we provide you with personalized care.

Order Test

Once we process your intake form, you will receive a lab requisition form via email. Please complete your lab testing at a time and location that is convenient for you. Quest and LabCorp have over 20,000 patient service centers for you to choose from.

Schedule a Tele-Med Visit

Typically, within 10 days of your lab work being drawn we can schedule you to meet with our providers for your new patient appointment. We will review your lab results and discuss your personalized treatment plan to help you achieve optimal health and wellness.

Therapy Consultation: Includes calls with our providers to tailor your treatment plan & ongoing future review appointments.
Lab Work: All of our treatment plans include Basic lab panels. (CBC, CMP, ESTRADIOL, PSA, TESTOSTERONE FREE & TOTAL) with (FSH & LH reviewed on initial labs)
Written prescription by our licensed providers for:
*Testosterone Cypionate: Up to 100mg a week or, for those who have adequate Testosterone levels you will receive a prescription for our Peptide Growth Blend or Peptide Recovery and Repair Blend
Shipping: Fees Included
10-Week Follow-Up: Post-initial consult, a follow-up visit is scheduled for 10 weeks later. You’ll be sent for labs around 8 weeks.
Treatment Adjustments and Refills: After the follow-up, adjustments are made as necessary, and a testosterone refill sent as needed.
Semi-Annual Reviews: Typically scheduled every 6 months for lab reviews and follow-up to ensure continuity and effectiveness of treatment.
Easy Reordering: Your prescription is sent every ~10 weeks, with adjustments based on lab findings.
