How can I start getting my primary care from Renaissance Physician Partners with Florence Health?
Call us at (832) 777-4884 or complete this form. You’ll speak with someone who can
answer your questions, and we’ll send you a Welcome Packet so you can tell us about
your health concerns and goals. It’s easy and we’ll help you every step of the way.
Who can be a patient?
If you’re age 65+ and have Medicare, you can — and all our services are
covered by traditional Medicare.
I need more than primary care. Can I see specialists?
Of course. If you need specialized care, such as a cardiologist or dermatologist, we’ll
refer you to a quality provider and help coordinate your care.
Do you offer visits by phone or online?
If you can’t come to the clinic — because you’re sick or don’t have transportation, for
example — you can consult with a provider by phone or video. Sometimes a face-to-face
visit is best, but telehealth is an option for many routine or minor medical issues.
Do you offer same-day appointments?
Yes! And if you’re concerned about changes to your health, our on-staff triage nurse can
give you the prompt attention and guidance you need. You and your health are our top
priority. You can count on us to be here when you need us.
